Theraputic Results of Total Anomalous Pulmonary Venous Return in Recent 7years

Abstract
This report provides follow-up data on 42 cases who were diagnosed total anomalous pulmonary venous return (TAPVR) at Sejong Heart Institute. Pucheon and Hanyang University Hospital, Seoul, excluding 11 cases associated with right isomerism.
The results were summarized as follows :
1. The patients were 26 males and 16 females and sex ratio was 1.33 : 1, ages at diagnosed of patients ranged from birth to 116 months (mean : 8.9 months) and weights at diagnosis of patients from 2.7§¸ to 20.0§¸(mean : 4.9§¸).
2. The anatomical types by Darling's classification were reviewed 22 cases (52.4%) in supracardiac, 15 cases(35.7%) in cardiac type. 1 cases (2.4%) in infracardiac and 4 cases (9.5%) in mixed type.
3. Mean pulmonary arterial pressure/systemic arterial pressure (PP/SP) was 0.63¡¾0.34.
4. Mean PP/SP of patients under 3 months of age was 0.68¡¾0.50 and that of patients over 3 months of age was 0.50¡¾0.35 but there was no significant statistical difference.
5. Among 42 cases, 36 cases underwent repair of TAPVR, 19 cases had supracardiac, 13 cases cardiac, 1 cases infracardiac, 3 cases had mixed type. Survival rates were 68.4%, 84.6%, 100.0% and 66.7%, so overall survival rate was 77.8%.
6. The survival rate of patients operated under 3 months of age and that of patients operated over 3 months of age were 72.2% and 77.7% but there was no significant statistical difference between two groups.
7. Among excluded 11 cases with right isomerism, 6 cases had supracardiac, 1 case cardiac and 4 cases had infracaridac TAPVR.
From this data, we concluded that early diagosis and early surgical approach was the key factors improving the prognosis of patients with TAPVR.
